Jail staff are supposed to report any “extraordinary or unusual occurrences” to the Jail and Detention Standards Unit (JDSU) within the Illinois Department of Corrections (IDOC). But jails have been haphazardly reporting for years, and some incidents were never disclosed to the JDSU.
